NJ Medicare B New, “Wellness Program” Benefit information:

SMAC Low Cost Blood Test for adults 18 years and older
* November 1st by appointment 7 AM to 9:30 AM
* Cost is $30. Prostate blood test (PSA) for men is $35. additional
* In person registration begins on October 11th at Health Department
Influenza Immunization Clinic ~ Seasonal Flu Shots
When: September 29th, Thursday
Time: 2:00 PM to 7:00 PM
Where: Elmwood Park Municipal Building
Cost: Free for those with proof of Medicare Card Part B. Must bring your card with you the day of the clinic please. $25.00 for adults without Medicare B.
Who: All adults 18 years and older
How: Walk in. Pre registration is not required.
Low Cost Blood Test, SMAC
When: November 1st, Tuesday
Time: By appointment 7:00 AM - 9:30 AM
Where: Elmwood Park Municipal Building, Court Room
Cost: $30. PSA is an additional $35. for men if requested.
Who: All adults over the age of 18 years old
How : Pre registration and payment is required.
Registration : Begins on October 11th. The Health Department is open from 8:30 AM to 4:30 PM daily.

Adult Health Consultation
Blood pressure and health consultation on the 3rd Tuesday of each month from 9:00 AM to 11:00 AM at the Elmwood Park Municipal Building, 182 Market Street. For further information, please call 201-996-2038. Services provided by the Health Awareness Regional Program (HARP) of Hackensack University Medical Center in conjunction with the Elmwood Park Health Department.
Child Health Conference
Vaccines are provided for school age children and Well Baby Visits on the first Monday of each month, 5:00 PM to 6:30 PM, and the first Thursday of each month, 9:00 AM to 10:30 AM, at the Elmwood Park Health Department, 182 Market Street. Influenza Immunization Clinic
The Elmwood Park Health Department conducts its annual immunization clinic for influenza in the fall. Pre-registration is required. All programs will be held in the Municipal Building. There is a charge of $20 for the flu vaccine. Medicare Part B participants must bring their Medicare I.D. cards to register. Any questions, please call the Health Department at 201-796-1072.
SMAC 23 Blood Screening
The Elmwood Park Health Department conducs its SMAC 23 Blood Screening in the fall. The test will check such factors as cholesterol, triglycerides, complete blood count and blood sugar. The screening also includes high and low density lipids (blood fats), an important heart risk factor. The fee for this test is $25. Optional for men only is the prostate (PSA) for an additional $35.
Advance registration and payment is required. Upon registration, all participants must provide the full name and address of their personal physician. The Elmwood Park Health Department urges those between the ages of 18 to 55 to participate in this screening. For additional information, please contact the Health Department at 201-796-1072.
